What is RESTORE2™

RESTORE2™ stands for Recognise Early Soft Signs, Take Observations, Respond, Escalate.

RESTORE2™ is not an admission avoidance tool – it is a right care, right time, right place tool, right outcome tool. It is a system that helps you to assess how unwell your resident is using six easy measurable observations so you can get the right support quickly.

  1. Breathing rate
  2. Oxygen Saturations
  3. Temperature
  4. Systolic Blood Pressure
  5. Heart Rate
  6. Level of alertness using ACVPU (Alert, Confusion, Voice, Pain, Unresponsive)

The Southwest nurse facilitators (Nicki Rogers and Maria Garrett-Marley) can offer free Restore2 training to care/nursing homes and provide RESTORE2 charts.

Care homes need to have completed physical observation training or have been recording physical observations.  Nurse facilitators will then complete RESTORE2 audits to ensure compliance and on-going support when required.

The home will receive a certificate declaring that the home has successfully past three RESTORE2 audits and RESTORE2 has been embedded in their home.
